Thank you for choosing to consult our office for your chiropractic heath care needs. You have made a great choice and it is an honor for us to serve you. To expedite your first visit to our office and to allow us more time to focus on you, we require that your choose ONE of the three following options:
- Call 1-866-400-8480 to schedule a COMPLIMENTARY consultation. We offer a 10-minute complimentary consultation by phone to see if you are a good candidate for care in our office. This phone consultation will allow you to learn more about the care we provide and allow us to determine if you are a candidate for an Initial Examination in our office. If we agree that you would benefit from care in our office, we will offer you an appointment at the end of the consultation.
- Call 1-866-400-8480 to schedule an appointment, then download and complete our required office forms PRIOR to your appointment. Download the following forms and complete them PRIOR to your first appointment. If you like you can email the forms back to us and we will print them out for you. If you are unable to download the forms, please call to request that we fax or email the following office forms to you so you can fill them out before your first visit.
Please note that all paperwork must be complete when you arrive for your appointment unless other arrangements were previously made with our staff. Otherwise, we reserve the right to re-schedule your examination to a later time.
- Call 1-866-400-8480 to schedule an appointment and arrive 30 minutes EARLY to fill our required office forms. If you prefer to fill out our office forms on your first visit, you will be required to arrive 30 minutes PRIOR to your appointment time to fill out all applicable intake office forms.
